St Mary's Centacare was founded in 1996 to meet the needs of the community by providing Aged Care Packages. Formerly known as CACPs, today the service runs four programs and our community support workers provide quality care to the South Burnett including the towns of Proston, Murgon, Wondai, Kingaroy, Kumbia, Maidenwell, Nanango, Yarraman and Blackbutt.

Our services include: Community Aged Care Packages provide individually tailored and flexible in-home care for up to six hours per week. Clients must be assessed and approved by an Aged Care Assessment team to receive this service Home and Community Care provides Domestic Assistance and/or Social Support to the frail aged or young people with a disability and their carer We are brokered by Veterans Home Care (VHC) to provide domestic services, respite and personal care Responsive Connections provides goal-based social support for adults with a diagnosed mental illness
